TECHNIQUE "Heparin less" Venovenous Bypass for Renal Cell Carcinoma Involving the Inferior Vena Cava: A Case Report
نویسنده
چکیده
Abstrac~-------------------This presentation offers a simple yet effective method of venovenous bypass without systemic heparinization to assist in the surgical intervention of renal cell carcinoma involving the inferior vena cava. This system is comprised of a closed BioMedicus (a) centrifugal pump system and cannulation sites of femoral vein and axillary vein. No direct cardiac cannulation is made. Renal cell carcinomas involving the inferior vena cava have been surgically treated by means of cavotomy, often with major loss of blood (1). Risks also include embolization to the right heart and lungs, poor visualization with less than desirable results, uncertainty as to retrieval of tumor thrombus, and the possibility of decreased return of venous blood to the right heart and those sequelae. Other surgical measures have included cardiopulmonary bypass with or without circulatory arrest. Also, a venovenous bypass from femoral vein to right atrium has been conducted (2, 3). Some complications associated with these uses will be discussed in the following text The degree of inferior vena caval involvement in this case report extended to the diaphragm and was handled surgically with the assist of this venovenous bypass system, but cases involving thrombus above the level of the diaphragm might also be considered depending on the extent and surgeon discretion. The bypass described herein affords a simple, convenient method to offer hemodynamic stability in the surgical procedure without cannulation of the heart and systemic heparinization. This bypass will be utilized in future cases based on individual situations.

An uncommon and insidious presentation of renal cell carcinoma with tumor extending into the inferior vena cava and right atrium: a case report
BACKGROUND Renal cell carcinoma is a potentially lethal cancer with aggressive behavior and it tends to metastasize. Renal cell carcinoma involves the inferior vena cava in approximately 15% of cases and it rarely extends into the right atrium. A majority of renal cell carcinoma are detected as incidental findings on imaging studies obtained for unrelated reasons. At presentation, nearly 25% of...

ntroduction. A combined abdominal and thoracic surgical approach is the treatment of choice for renal cell arcinoma with secondary thrombus extending to the supradiaphragmatic vena cava and initially into the ight atrium. This procedure usually requires a median sternotomy with cardiopulmonary bypass and deep ypothermic circulation arrest or, alternatively, venovenous bypass. In this report, we...
متن کاملSurgical technique for the treatment of renal cell carcinoma with inferior vena cava tumor thrombus: tips, tricks and oncological results.
Renal cell carcinoma represents 3 % of all cancers. Around 4-10 % of cases present with inferior vena cava involvement, generally with tumor thrombus. Clinical and preoperative stage will be classified depending of the thrombus extension. A high quality preoperative workup is essential to properly plan surgical approach. Complete surgical resection of the tumor is potentially the only curative ...
متن کاملEfficacy of pre-surgical axitinib for shrinkage of inferior vena cava thrombus in a patient with advanced renal cell carcinoma.
The authors present the first case report of pre-surgical axitinib treatment on primary renal tumor and vena cava thrombus. We report the case of a 78-year-old woman with renal cell carcinoma and inferior vena cava tumor thrombus, successfully downstaged with pre-surgical therapy with axitinib. A significant objective response was observed for tumor size and thrombus. After initiation of axitin...
متن کاملSarcomatoid Variant of Urothelial Carcinoma of the Renal Pelvis with Inferior Vena Cava Tumour Thrombus: A Case Report and Literature Review
Sarcomatoid variant of urothelial carcinoma (SVUC) of the renal pelvis is a rare entity. To the best of our knowledge, around 25 cases of this neoplasm have been reported in the literature to date, most of which were of high stage. The inferior vena cava tumour thrombus, which is a hallmark of renal cell carcinoma (RCC), may rarely be found in urothelial carcinoma of renal pelvis. In this repor...
